Whether your child is coming to Children’s Hospital for a CT, an EEG, or blood work, we want to make sure you have all the information you need to help making being here a positive and pleasant experience for you and your child.
Your child’s doctor will tell you on what day and time your child is scheduled for his or her outpatient visit or procedure. Please discuss with your child’s doctor any special steps that must be taken before admission (i.e. blood work, medications, etc.). You will also want to clarify with your child’s doctor at which Children’s Hospital facility your child will be having his or her procedure.
To begin the admission process, you will first go to the Business Office. The Business Office will ask for information about you and your child such as name, address, birthday, social security number and possible health insurance coverage. If you have any identification cards or health insurance cards, be sure to bring those with you to your outpatient visit.
You will also be asked to sign forms that permit the medical center to perform tests and provide treatment. At this time, you will also receive information about insurance coverage and the billing process.
The following list will help you remember items and information to bring with you to your child’s outpatient visit.
