 New Patient Process

All new patients must be initially referred by their primary care physician before they can be seen at Developmental-Behavioral Pediatrics. PCPs can access the referral form here:

Before your child is scheduled for an appointment, you must complete the new patient packet, which includes both demographic & insurance information as well as the parent questionnaire. If your child is in school we also ask that you have your child's teacher(s) complete the teacher questionnaire and request records from the school.

Once we have received the new patient packets and questionnaires, our social workers will review all information to determine the best appointment type for your child.

Unfortunately due to high demand, we do have a wait time of 2-10 months (depending on child's age) before you will be able to be scheduled for your appointment.
